MEDICAL RELEASE FORM:

I hereby allow (child’s name) to participate in the Alief Futsal League. I know of no mental or physical problems which may affect my child’s ability to safely participate. I realize that futsal is a physical activity involving contact. I therefore accept responsibility for my child’s physical condition. I hereby promise and agree that I will not hold Alief Futsal League nor its employees liable for any loss, damage, or personal injuries received as a result of participation. I hereby authorize the directors and staff of the Alief Futsal League to act for me in accordance with their best judgment in any emergency requiring medical attention. Furthermore, I hereby release the Alief Futsal League and its agents and its staff from any and all liability for any injuries or illnesses that may arise during the Alief Futsal League. I understand and assume the hazards and risks associated with this activity and waive all claims against the Alief Futsal League and its staff.

PHOTO AND VIDEO RELEASE CONSENT FORM:

I hereby grant permission to the Alief Futsal League to use my image on its website, Instagram, or Facebook page without further consideration. Additionally, I grant permission to use images of my children listed below. I understand that no names will be used on the website unless specific written permission is given to an appropriate staff member. I acknowledge that once any image is posted on the Alief Futsal League’s website, Instagram, or Facebook, it can be downloaded by any computer user around the world. I hereby attest that I am the legal parent/guardian of the child(ren) listed below. This consent is effective until such time as I revoke it in writing and provide a copy of the revocation to the Alief Futsal League.

FAN CODE OF CONDUCT:

We strive to create a family-friendly environment where fans can enjoy the game free from disruptive, inconsiderate, or illegal behaviors. To ensure a positive experience for everyone, we ask that fans adhere to the following guidelines:
Positive Cheering: Fans are encouraged to be loud, proud, and positive by cheering for their team, not against the opponents or officials.
Respectful Behavior: Fans should refrain from unruly conduct, including profanity, abusive language, obscene gestures, racist comments, offensive remarks of a sexual nature, trash-talking, taunting, heckling, fighting, intimidating actions, or verbal or physical harassment directed at officials, student-athletes, coaches, team representatives, or other spectators.
Impairment: Fans appearing intoxicated or showing signs of impairment that result in irresponsible behavior will be denied entry and/or removed from the facility.
No Smoking: Fans shall not smoke or use tobacco products of any kind.
No Throwing Objects: Fans should refrain from throwing objects and/or possessing prohibited items.
Restricted Areas: Fans are not allowed inside the playing area unless escorted by authorized personnel.
Compliance: Fans shall comply with requests from game day personnel and public safety officials regarding facility operations and emergency procedures.

If you choose not to respect our code, you can expect the following:

A verbal warning and/or
Removal from the facility by an administrator or public safety official without a refund.
